Person Centered Approach to
Care & Planning
The services and supports provided by Peel Home & Community Supports are directed and controlled by the person, their families, carers or any other person the individual wishes to have participate.
Together we will explore the person’s individual needs and goals then work in partnership with them to achieve the outcomes they want for their lives.
All of our support services are responsive to the changing needs of the individual. These can be delivered in the home, in the community, or through a customised, flexible support plan, tailored to your goals, daily routine, individual preferences and daily needs.
Some examples of these may include (but are not limited to):

Our Services
Assist Personal Activities
Assist a person to undertake and/or develop skills to maintain their home environment & live autonomously.
Assistance with attending activities such as to:
Arts & Craft classes
Educational / Vocational classes
Sport & Recreational activities (swimming, fitness & gym classes, etc.)
Hobbies & Pastimes
Household & Domestic Assistance
We provide assistance with essential house cleaning that a person has difficulty in doing for themselves, such as:
- Domestic cleaning; bathrooms, kitchen, lounge and bedroom areas
- Assistance with laundry
- Assistance in making beds
- Assistance with meal preparation
Transport/Travel
Providing support to enable people to help build their capacity towards independence with travel or to provide training for a person to be able to use Public Transport such as:
- Trains
- Buses
- Ferries
To assist with transport arrangements for attendance at:
- Doctor’s appointments
- Occupational Therapy appointments
- Health Care appointments and/or assessments
- Scheduled visits to hospital for appointments or,
- Other health related appointments
Assistance can also be provided to attend Personal Activities, or to assist with shopping, where the person has limited capability in achieving this independently.
Plan Management
Our role as your Plan Manager is to assist you by paying your supports to providers from your NDIS funds. Allowing you to live your life while we manage your claims, payments and administration of your NDIS budget.
Attendant Care/Personal Care
As a client you are involved in all aspects the planning and design of the support they want.
You will have choice and control to determine the time of day and day of the week you wish your support to be.
Peel Home and Community Supports will provide compatible support staff who are experienced and trained and who will treat you with dignity and respect, and will work with you to achieve your goals.
All our staff will be respectful of your individual belief system or culture.
By providing our services we aim to support people so they can remain independent and that they can make personal life choices, live in their own home, stay connected to their families, friends and community.
Personal care can also include assistance with:
- Showering & Bathing
- Personal Hygiene
Together we will work collaboratively with you to establish your support network and have choice and control over your independence.
Community Access
Community access needs to be flexible to allow for the people’s changing needs, there no one-size-fits-all method to improve the quality of life that the person wants, this will vary according to the challenges the individual is facing.
Our Organisation will assist in providing you with community access for your day-to-day support on a one-to-one ratio with an age-appropriate support worker to participate in the activities of the person’s choice.
Our Organisation can provide you with skilled support workers who will ensure that learnt life skills are enhanced and built on with time taken to upskill you at a pace that suits your choice.
Our Organisation will ensure that you have access to community activities, educational and vocational activities, transportation, information and communication to assist you with an equal opportunity to participate.
By dismantling barriers enables you to build skills and take an active role in your life’s development.
Should you choose to attend an educational or vocational facility, a support worker, can accompany you to support and assist you for as long as they choose.
There are a variety of TAFE institutions within the surrounding areas that offer a multitude of different learning opportunities for people.
Finding and Keeping a Job
We are here to help you learn new skills and get ready for work. We’ll connect you with different businesses across Mandurah and surrounds for work experience and job opportunities.
We can also help you prepare for work activities, brush up on your reading and maths skills, help you to get your driver’s licence, and obtain essential qualifications, resume building. Just talk to our Admin team, and let’s get started on your journey to success!
A private service, often referred to as Fee-For-Service, is used by individuals seeking assistance in their homes or communities, which they finance independently.
Private services are available to individuals supported by the Department of Communities, the Department of Health. Additionally, self-funded retirees are also eligible for private services.
